If you are sure a Public Record is what you are seeking, use the button below to open a new page allowing you to choose from a wide array of records maintained by the Arizona Department of Corrections. You may also use the page to fill out a large text box to request a public record that does not appear as a selection. Public Access will assess the feasibility of your request. You will be prompted to specify the type of record you are requesting from the following options:

How to pay for your Department of Corrections public record request:

After requesting your record or records, click the button below to use our online payment system, where the record request can be specified and an appropriate payment submitted. Refer to the following table of standard charges for the price of each record or media file that you are requesting:

Document Cost
Copies $.50 per page
Subpoena $.25 per page plus $25 per hour for collection and processing
CDs or Digital Uploads $15.00 (additional $2.00 for shipping CDs)

How to receive your requested document:

The application that is used to request your public record will prompt you to input a valid email address so Public Access can communicate with you concerning your request and/or upload responsive records for release to you. A physical address or P.O. Box can also be specified to receive paper copies of your request.
